Applying for subsidised care with Anne Health
1. Overview
Anne Health is committed to providing essential, gender-affirming healthcare to trans, non-binary and gender diverse people who face financial barriers to accessing private care.
Through our crowdfunder and Anne Health’s own contributions, we are offering:
Five fully funded Anne Health Standard Membership places (100% free subscription)
Five partially funded places (subscription reduced to £50/month)
By applying for this funding, you agree to the following Terms and Conditions.
2. Eligibility criteria
To be considered for funding, applicants must:
Be trans, non-binary or gender diverse.
Be a resident in the United Kingdom.
Demonstrate financial need preventing access to private gender-affirming healthcare.
Not have previously paid for private gender-affirming healthcare
If under 18, have parental or legal guardian consent.
3. Application requirements
Applicants must submit the following supporting documentation:
Evidence of financial need (e.g. a benefits statement and/or two months’ worth of bank statements dated within the last 3 months).
Proof of photo ID (passport, driving licence, etc.). If your ID is in an old name, you will also need to provide proof of name change (e.g. a deed poll document).
Proof of address (dated within the last 3 months, e.g. bank statement, credit card bill, utility bill).
For applications on behalf of someone under 18, a child benefit statement (within the last year), and their ID documentation.
Incomplete applications, or those missing required documentation, will not be considered.
4. Application process
Applications must be submitted through the official online form.
The form will open on Thursday 10th July 2025 at 8:00GMT and will automatically close after 30 submissions to ensure fair processing.
Applications will be assessed on a first-come, first-served basis for those meeting eligibility and documentation criteria.
All applicants will be notified of the outcome within 5 days of the form closing.
5. Selection and allocation
Successful applicants will receive either a fully funded or partially funded Anne Health Standard Membership place.
Funding preference (full/partial/either) will be considered but cannot be guaranteed.
Allocation decisions are final and at the discretion of the Anne Health grants team.
6. Scope of funding
Funding covers Anne Health Standard Membership services, including clinical consultations and care coordination.
Funding does not cover:
Medication costs
Travel expenses
Surgery or surgical assessments
Other healthcare providers’ services
General living expenses
Other private medical treatment
7. Verification and consent
By applying, you confirm that all information and documents submitted are true and accurate.
Anne Health reserves the right to verify the accuracy of any information provided.
If you are selected for funding, you consent to provide any further necessary information to confirm your eligibility.
In order to start your membership, you will be asked to provide a copy of one of the following as valid proof of identity:
Passport
Driver’s license
National identity card
Birth certificate
Bus pass
Bank statement
Student card
Residence permit
Military ID
CitizenCard
Adoption Certificate
Marriage Certificate
If you have changed your name by deed poll, please provide a copy for our records.
You will also be asked to provide a copy of one of the following documents dated within the last three months to confirm your current address.
Utility bill (for example: electricity, gas, water, phone)
Bank or credit card statement
Council tax bill
Mortgage statement
Tenancy agreement
Official Government correspondence (e.g., tax letter, benefits letter, National Insurance letter)
Medical letter (e.g. GP letter, hospital appointment letter)
8. Confidentiality
All personal information provided in applications will be treated confidentially.
Your data will only be accessed by the Anne Health team members responsible for assessing applications and administering the funded care programme.
Data will be stored securely and processed in accordance with our Privacy Policy and applicable data protection laws.
9. Ongoing eligibility
Funded membership may be renewed as long as the recipient continues to demonstrate financial need and meets clinical requirements.
Anne Health may ask funded members to provide updated financial information annually.
If a recipient’s financial circumstances improve such that they can afford private care, they will be asked to step aside so funding can be reallocated to someone in greater need.
10. Applicant responsibilities
Applicants are responsible for ensuring they provide accurate, complete and timely information.
Applicants must respond promptly to any communication from Anne Health requesting further information or clarification.
Successful applicants will need to complete all required onboarding and verification steps before starting funded care. To continue receiving funded services, they must actively engage with our care and follow-up requirements. Eligibility for funded care will be reviewed regularly.
11. Decision-making
All allocation decisions are final. Funding is limited and high demand means not all eligible applicants can be funded this round.
Anne Health reserves the right to adjust timelines or funding distribution in exceptional circumstances.
12. Appeals
There is no formal appeal process for funding decisions.

13. Future rounds
Anne Health will seek to offer further funded places in future, subject to funding availability.
